Use this service to request an appointment for a cardiovascular disease review.

You can use this service if you:

• are registered at the surgery

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your preferred day and time for the appointment

Your appointment will last no more than 15 minutes with a member of our nursing care team. During this appointment we will:

• take a blood sample
• check your blood pressure
• record your height and weight
• record your waist circumference

A member of our clinical team will contact you about 7 to 10 days after your appointment to discuss the results.
